PROCEDURE FOR REGISTERING A COMPLAINT


POLICYHOLDER OR CLAIMANT MAY REGISTER HIS / HER COMPLAINT WITH RESPECT TO HIS POLICY OR CLAIM IN FOLLOWING MANNER FOR REDRESSAL OF HIS / HER GRIEVANCES.

   i)
            a) By submitting a written & signed complaint through by post at address given below:-

            Divisional Head (PHS-PO),
            First floor, PHS Division,
            State life Building No. 9, Dr. Ziauddain Ahmed Road,
            Karachi-75530.

            b) By written application to our Principal's Office Complaint Cell to :-

By submitting a written & signed complaint along with photocopy of National Identity Card to Zonal In-charges of PHS department or Zonal Heads. Addresses of all zones are available on the website and rare side of the policy document cover.

iii)
By calling at the given numbers of concerned zonal office available here on the website and to the rare side of policy document cover.
